Musiedlak, Thomas D. passed away Saturday, July 30, 2016 at the age of 74. Beloved husband of the late Kathleen. Loving father to Diane, Christine (Ron) Vick and Linda (Mike) Dorie. Proud grandpa of Jared, Hailey, Elijah and Evan. Dear brother of Joanne Borchardt, Geri Musiedlak, and Suzie Jarvis. Cherished uncle to Jeff Musiedlak. Preceded in death by his siblings Jerome (Judy), Kenny, and Carol Serio. Survived by nieces, nephews, other relatives and friends. Visitation Friday, August 5 at the funeral home from 4 to 7pm and on Saturday, August 6 at the funeral home from 10 to 11am with Funeral Service at 11am. Interment Holy Cross Cemetery.
